User error/carelessness is mostly the problem.

I'd be interested to know what the source is for that claim. (I'm talking captive bolt systems here).

"User error" is a difficult thing to define when there's a live animal that could be moving around "on the other end" but even so, in my experience, this is far from a regular cause of incomplete stunning. A captive bolt gun failing to work first time will cause no real harm to the animal. The issue would be if the gun worked but did not hit the "right spot". People who use these things in approved establishments are (or should be!) trained and licensed professionals.

While aware of the OP's query being limited to animal welfare in the Jewish and Muslim communities, may I take the liberty of taking the thread title at its face value and expand the discussion to include the traveller community?
In this case, reported yesterday on the BBC site, a traveller stapled and superglued his dog's face together in order to participate in hare coursing more than 85 miles from his home. Apparently as a member of the travelling community, hare coursing is ingrained in his culture and he was unaware that it was illegal. His poor literacy skills prevented him from understanding that he was banned from entering Lincolnshire as a result of a previous hare coursing conviction. Suspended sentence, of course.
