Perhaps you give her too much credit. It might have been a genuine error, more likely the only error was to think she could get away with it. Either way she's obviously not up to the job, or if she is she's lost the public's confidence.

Not sure that supports a "genuine error" and if it does, she has been incredibly reckless and cavalier over what duty she had to pay
Edit: Not sure if the reference to any other reasons in the HMRC guidance could apply to Rayner's situation
